Gynecomastia revision surgery, or corrective male breast reduction surgery, is not uncommon. For the majority of post-gynecomastia surgery patients who are unhappy with their initial surgery, this secondary surgery, while sometimes more complicated, is still an excellent option.

Often, corrective gynecomastia surgery requires more skill, expertise, and experience than the initial surgery. Each case must be individualized and carefully orchestrated. Scar tissue, crater-like deformities or other obvious indentations may require further tissue removal and sculpting, fat flaps, or fat grafting.

Top Two Reasons for Male Breast Revision Surgery

Reason #1
The initial surgery employed only liposuction, which did not reduce the breast gland tissue.

An important point to remember about gynecomastia is that the breast gland tissue is a major component of male breast development. The breast tissue may be hard and firm or composed of a softer fibrous network of tissue. In both cases, surgical removal of this tissue can provide an optimal outcome.

However, many surgeons perform gynecomastia surgery using the liposuction technique without removing the breast gland. The liposuction techniques that are utilized today include: ultrasonic liposuction, tumescent liposuction, smart liposuction, and sharp liposuction cannulas. While these treatments may be adequate for pseudogynecomastia, they do not remove the glandular tissue found with true gynecomastia.

Reason #2

Many plastic surgeons take a “do no harm” approach, in part because taking out too much breast and fat tissue is more difficult to correct. The surgeon will be in a better position to perform a revision if there is still sufficient tissue left to correct.
