Sculpting Your Chest After Gynecomastia

Elo everyone,

My name is David and I had Gynecomastia surgery in July 2009. My moobs weren't too bad before surgery (I had 40g of fat removed from the left and 30g from the right) but it still brought my confidence and self-esteem levels to a minimum. To this day my chest still looks a bit moobish looking and the doctor has confirmed it is just loose skin as a result of it all. I was hoping and preying that there is someone out there that can tell me if it is at all possible that my chest skin will get tighter and stretch out to a normal shape if I increased my muscle mass and lost body fat? Also can anyone recommend top scar removal creams as I also have noticable scars? Please help me someone I don't know how much more I can take of this.

You are now about 18 months post op.  What you see is what you will have -- no further spontaneous improvement can be anticipated.  Any skin tightening is already completed as well.

It would be best if you provided some photos for us to look at -- and also some info about yourself, such as height, weight, maximum weight, etc.

And by the way, there are no scar removal creams -- do not waste your money.  The scars that you have may improve with time or you may wish to consult with a plastic surgeon to explore the possibility of scar revision (to improve but not remove the scars) or a dermatologist who may be able to laser the scars and make them fade a bit.

Hi,

Thanks for the advice. To be honest I wasn't really anticipating top results anyway I've come to accept it. However I was to lose fat and add more muscle mass, would the contours of my nipples even out by just a fraction? Sorry I understand it is hard to give a concrete answer without pictures, unfortunately the only pictures I could upload will be shots I have taken myself. I will post some decent pics taken by another person sometime tomorrow. Although if you could just give a quick relieving comment I would be very gratious.

In general, if you lose weight and build up lots of muscle, it could theoretically take up some of the slack of lax skin -- but the changes are extremely individual.

That's what I predicted, however it is hard to get a concrete answer as I got mine done on the NHS and they don't have a specialised clinic in dealing with gynecomastia. I plan on posting various pics of when my chest is relaxed, side views, pics of when I tense (when I tense I get this floppy skin look, especially on my left chest due to where the gyne had developed and shaped my chest from an early age), along with my height, weight, maximum weight, and my training plan on adding muscle mass, just waiting for my mum to get home! Nonetheless thank you so so much Dr Jacobs, I can't begin to tell you the relieve you are bringing to my overactive brain :D

Sorry big judging by KB, its says maximum size file can only be 182kb and each picture is about 3.5 times larger than that is allowed. Is there an E-mail address I can send to or anything else?

Delly    Thanks for the photos.  Obviously, without a hands-on physical exam, there is a limit as to what I can discern.  But my gut feeling is that your original operation was incomplete and that there is residual tissue there -- that is, there is not simply loose skin. 

If you were to lose some weight, it would probably improve -- but I doubt that it would be a trim, taut, contoured chest without an additional revision operation to remove some more tissue.

Best to return to your original surgeon to discuss.

Well I have a miniscule amount of gland tissue left in either, compared to before the operation, which I was told so it wouldn't cause inverted nipples. The tissue left in there is barely noticeable to the touch, which is obviously an improvement. Is it possible (going back to losing body fat and increasing muscle mass) that the contours of each nipple would surface a little better through diet and training? I do however have 180 degree scars on each nipple bit more of a noticable scar on the left. The scar on the left turns into a deep sharp crevice when stimulated however the scar on the right is more of a skin tone problem not really noticeable unless you look closely. I was planning on getting a couple of tattoos on my chest near that region so I am not so bothered about the left, its just the right one with the deep crevice that bothers me! I was thinking about maybe getting some sort of skin resurfacing done just to improve the look of the scars, obviously before I get any tattoos done! Could you possibly advice on what to look for in a clinic when considering skin resurfacing an option? And if I am being ripped off over the price? I really want to have revision gynecomastia surgery as an absolute last resort but I would be happy if the scar on my right nipple improved.

There is absolutely nothing lost by continuing diet and exercise.  That can result in more muscle and less fat on your chest and perhaps improve the overall situation.

Ah Doc I sincerely can't thank you enough mate. You have shed more light on the subject than anyone I have spoke to over here, and you have given me the motivation to carry on! There is only one thing left to do... My journey starts now.

JJJust one more thing and I'll be out of your hair haha, I have been looking at this clinic online for Skin Resurfacing (Cambridge Skin and Laser Clinic) and it looks decent and trustworthy. Thing is I ain't got a clue how to tell good clinics from bad ones haha it shows you pictures of the doctors, 17 years worth of experience, they tell you the equipment they use, techniques they use, pictures of inside the clinic, everything. Is there something I should look out for when doing my research? What could go wrong with Skin Resurfacing?
