Hey to all out there suffering through the puffy nip problem.  I've found a surgeon I can trust, but I figured I ask here to get some anecdotal advice as well.

My prob is essentially puffy nips only.  On the right side, I can't feel a lump beneath the nipple anymore, but the nip gets puffy when warm or out in the sun.  On the left side, there is a small rubbery gland beneath the nip.

The doc gave me steroid injections beneath the nipples.  He thought my case was minor enough to get rid of without surgery.  The glands have reduced in size, and there is less puffiness.  However, I'm ready to get this done once and for all.

The feeling of never having to check or rub your nipples to make them look normal must be a huge relief.  Also, I'm in good shape, which actually makes the puffy nipple problem more bothersome.  Before I put on a little muscle (I used to be super skinny), the problem never bothered me much.

My main question is will excision surgery remove the puffy nipple/protrusion problem?  I can post photos if that helps.

My main concern is whether surgery gets rid of puffy nips or if puffy nips are just something some guys have to live with.  Hopefully not the latter.

1. Puffy nipples are ALWAYS due to breast tissue (gland) directly under the areolas -- fat just doesn't do this.  
2. Surgery is the only way to remove the glands and deal with puffy nipples -- permanently.
3. One need not try to feel for the "gland" -- it is not necessary.  Further, some gland can feel soft and some fat can feel firm.
4. There is no other treatment for puffy nipples -- Botox, creams, massage, hormone manipulation, injections, etc do not work.
5. Puffy nipples -- especially on a slender guy -- can be tricky to deal with surgically.  Too much removed and you have a crater and not enough removed and you have residual puffiness.  Sometimes I spend as much or more time in surgery for "minor puffy nipples" than I spend on a large gynecomastia case.  Make sure you go to a gyne specialist for this surgery -- it will be worth it.
6. Finally, if the surgery is done properly, then your puffy nipples will be gone and the areolas will lie flat and flush with the surrounding chest skin -- from day one after surgery.

Wow, that's great info.  Thank you, Dr. Jacobs.  I'm going to talk to the doc more in-depth.  he did mention that with surgery, he would leave a bit, so the nipples don't concave.  It's Dr. Benny Herbosa from Manila.  He's done a lot of surgeries for men on here.  He's a great guy in person too.  He's performed hundreds of gyne surgeries.  

The steroid injections do help, but they don't get rid of the problem completely.  He said he's had a few patients who were content with the results of the injections alone.  I bet those guys weren't into weightlifting.

I'll pass along this info to him to see if he's confident with the procedure.  Flat nipples sound like paradise :) I'm a surfer and like to go to the beach a lot.  Puffy nipples can also thwart a weight training program.  It's great to see gains, but the eyes always travel back to the puffy nipples.

I'm 33 now and have had these since I was 12.  One thing, though, they have never really affected my confidence with the opposite sex.  They've been more of an annoyance than anything else, and like I said, once a man gets into weightlifting, it actually pushes them out.

I'd like to get more aggressive with my training once this is all in the rearview.
