Before Surgery Questionnaire:
Did your surgeon need to uncover a medical problem to stabilize before surgery?

Did you need to see an Endocrinologist?

Are you a bodybuilder or athlete?

Do you need to have your chest exposed during your sport?

What did you use to camouflage your chest contour issues?

Post Surgery Questionnaire:

Did your doctor measure or talk to you about your body fat?  Did you lose weight before surgery, if so how much?

How long have you been looking into gynecomastia surgery before you had the operation?

Do you feel your doctor pressured you or rushed you into surgery?

Did you travel for your surgery?  If so how far? Why?

Were your breasts sagging?  If so how much: None, a little, a great deal, sagging around the entire chest

change surgery options to select all appropriate and use buttons: liposuction, excision, ultrasonic, vasor, chest lift, other with fill in

How many incisions on each side?

Are the scars well hidden?  Hidden, Show a little, Still Evolving, Show somewhat, Obvious

While looking in a mirror from how many feet away do you have to be before you can see your scars?

How does the result look flexing your muscles, arms up over your head, and during activities?

How many operations were needed to achieve your results? Did you anticipate this number if more than 1?

Do your results match what you saw of your doctor either on the web or in his / her office? Yes No My Doctor did not have before / after pictures

Did you need drains?

Did you need to use the prescription pain medication? If so how many? How long after surgery?

How long after surgery are you?

Did you have swelling after surgery? None, A little, moderate, quite a lot
If you had swelling how long did it take to resolve? None, A few days, A week, 2 weeks, 1 month, 2, 4, 6, still swollen

Did you have bruising after surgery? None, A little, moderate, quite a lot
If you had bruising how long did it take to resolve? None, A few days, A week, 2 weeks, 1 month, 2, 4, 6, still bruised

Compression After Surgery Choices extended to None, One Week, 2, 3, 4 or more
Type compression check all:  None, Ace, Binder, First Stage Garment, Second Stage Garment, Before Surgery Body Shaping Compression Garment

Hypersensitivity after surgery None, Yes but resolved, yes still present

How soon after surgery did you return to work and what was that like?

Have you had recurrence or regrowth after surgery?

What is your age? Height? Weight?

How long have you had this condition?

What first brought your attention to the condition?

How is the condition impacting your life?

In what way have you attempted to conceal the condition?

Are you aware of any closely related male family members who also have gynecomastia?

Have you discussed this issue with a health care professional? If so, what kind?

Have you been taking any kind of medication, vitamin or supplement? If so, what and how much?

If you consume alcoholic beverages, describe your drinking habits.

If you have used any street drugs, describe your drug usage.

You could ask those who have had corrective surgery how the procedure was paid for. For example: Mom/Dad, other family member or by their own means... For the person(s) that paid, was it by credit card, bank loan, cash etc...

How much was the final surgery cost...

For those that still have G and want it removed, is the issue of the surgeons fee the only thing holding them back...

Pre Operative Questions
1.why did you choose to have surgery now?
2.How did you go about finding a surgeon and choosing a surgeon?
3.Did you have additional testing of your hormone levels or bleeding test?
4.Did you go out of state for surgery?
5.did you discuss this with love ones or was it top secret?
6. were you well informed going into surgery.
7.How bad was your condition?
8 Is this your first surgery for gynecomastia?
Post operative Questions

1. were you happy with your experience. If not, why?
2.what type of anesthesia did you have?
3.did you have drains, how long?
4. did you wear a garment? How long?
5. was your tissue sent to pathology. was it normal gynecomastia tissue?
6.was it painful and uncomfortable experience?
7. how long until you went to work, drove a car, or exercised?
8. did you loss any nipple sensation.
9. how long since your surgery?
10. were your expectations met?
11. would you recommend to a friend?
12. how long did it take you to expose your chest comfortably in pubic after surgery?
13. how long was your post op swelling present?
